My dog's nose is raw and getting worse from licking. What should I do?

A few days ago I noticed my dogs nose is becoming raw. Since then he keeps licking it and it’s getting worse. Around the same time I found worms in his poop. He’s being treated and is doing better with the worms. Pet's info: Dog | Mixed Breed Large (61lb +) | Male | unneutered | 1 year and 7 months old | 70 lbs
