How to stop my cat from peeing outside the litter box for good?

My cat, cocoa, won't stop peeing outside of the litterbox (even though I clean twice a day) and its gotten to the point where I will have to get rid of him. Please help. Pet's info: Cat | Mixed Breed | Male | neutered

Poor guy. It could be that Cocoa simply has a urinary tract infection or another condition called feline lower urinary tract disease.
