My turtle is sick from only eating leaves. What should I do?

I have got a turtle which I have been feeding only cucumber and leaves for about a year.. So its skin is beginning to be cut actually and has a very very bad vision and I tried actually to feed it carrots but it found it really rough and tough to eat.. So what am I supposed to do? Pet's info: Amphibian | Unknown - Amphibian | Male | 5 years and 1 month old
