If you are able to handle the cat you can try using this wound gel topically every day until it is healed. ( https://www.petco.com/shop/en/petcostore/product/vetericyn-cat-wound-and-skin-hydrogel ) Honestly, antibiotics (either oral or injectable) may be needed to get this to clear up so if the wound gel does not work then you need to get him to a vet for an exam.

    Unfortunately if it has not responded to standard medications and no topical options are practical, further investigations under sedation are required in order to determine the underlying cause and guide the most appropriate treatment. A skin scraping and biopsy would be recommended in order to assess if this is the result of or even mixture of an auto-immune, parasitic, fungal or bacterial issue
