Why is my old dog having accidents and acting miserable when alone?

Pet's info: Dog | Mixed Breed | Female | spayed | 12 years and 7 months old | 60 lbs

My dog is 12 years old and very well trained. There are a couple of things that have me worried. First is that she is starting to pee and poop in the house everyday at least 3 times a day. She has never done this before. She just peed right in front of me. She knows better than peeing in the House. She acts healthy otherwise. She will play walk and run fine. Her behavior is fine when I’m around as we’re in her main owner but as soon as I leave my gf says the dog is miserable.

I recommend to have Hannah examined by the veterinarian for a urinary tract infection. This could be the cause of the symptoms. Also monitor her for any signs of dementia.
