It's unlikely that a single dose in a healthy dog would cause internal bleeding, but aspirin is a blood thinner, and in dogs on certain medications or with certain health issues, one dose could cause fatal bleeding problems. It is why most vets never recommend its use in dogs with unknown causes of pain.
