My dog has worsening hot spots on her legs after fleas. Vet needed?

Pet's info: Dog | Mixed Breed Large (61lb +) | Female | spayed | 3 years and 2 months old | 80 lbs

Is this just a hot spot or something from her scratching that can just be treated OTC with HC cream or something? Or could it be worse and need to be seen by the vet? She recently had a few fleas and was scratching a bunch but I gave her one capstar and she hasn’t seemed to scratch much recently, but these spots look like they’ve gotten worse. They’re on both sides of her inner back legs but one side is worse than the other. HC seemed to help in the past on a dif. spot that didn’t look as worse

So Capstar is a one time pill that starts killing fleas in 30 minutes, but doesn't provide any lasting protection. I suspect it's fleas causing Marley's problems. I would take her into a vet today to get treated.
