My dog is anemic, lethargic, and prednisone failed. What should I do?

Updated On September 23rd, 2025

Pet's info: Dog | Goldendoodle | Female | spayed | 7 years old | 40 lbs

My dog Elly, 7, has been lethargic and not eating very well for about a month. We took her in for blood work and her red count at 25 and platelets were up bt wbc count norm. she also got an x-ray, abdominal ultrasound, and an echo-cardiogram showing no abnormalities other than a mild flow murmur. She has been on prednisone for 6 days now and today she got blood work and her red count is down to 18. We are upping her prednisone dose and checking again tomorrow but we have no clue what to do.

Thanks for using Petco Pet Education Center, formerly Petcoach! This may be the time for your veterinarian to check in with an internal medicine specialist as this is a bit more complex than typical. Possibilities include an immune-mediated hemolytic anemia in which case other immunosuppressive drugs can be used with prednisone or possibly an infectious disease. They would be able to recommend further but reaching out to a specialist would be an option for further tests/recommendations. Hoping for a speedy recovery!
