Why isn't my dog's oozing leg infection healing with ointment?

Beneath my dog rear leg there is some infection going on and blood with fluid in small amount is oozing vet gave ointment and I am applying miconazole spray and ointment but not much improvement kindly see the attach photo and tell me what is it and its cause. Pet's info: Dog | German Shepherd | Male | unneutered | 3 years and 7 months old

I am sorry to hear about your dog. If this is not improving with topical medication he may need to be switched to oral/systemic medications as the problem may be deeper under the skin than topical meds can penetrate. I would let your vet know he is not improving. Occasionally we may need to do testing such as cultures or biopsy in some cases that don’t improve with traditional treatments.
