My dog has a growing ear hematoma and is lethargic. What should I do?

I’ve noticed my dog has an ear hematoma. It keeps getting bigger and now when we try to pick him up to help him into the bed or on the couch he yelps. I’ve also noticed him being a little lethargic. He’s really usually energetic. Pet's info: Dog | Beagle | Male | unneutered | 8 years and 3 months old | 21 lbs
