My dog has an ear infection and is waking up and panting heavily. I gave him a drink of water and then he seemed okay but about an hour later he started to do it again.

He may be having a lot of pain when he wakes up doing this. If he hasn't yet been to a vet he needs to be seen as soon as possible. If the pain makes him shake his head, he could shake it too hard and break a blood vessel in his ear and cause a hematoma which needs surgery to correct. Plus, ear infections hurt, and he needs relief as soon as possible. Your vet will do an exam, labwork and will probably prescribe antibiotics and drops for the ears.
