My dog has a head lump for a year, now with a black hole. What is it?

Pet's info: Dog | German Shepherd | Male | unneutered | 2 years and 7 months old | 88 lbs

My dog has had this double lump on his head for over a year now. Today there appears to be a black hole. I took him to the vet and was just given antibiotics. No real explanation of what it is Please see attached pictures.

This is concerning. Looks necrotic (like dying tissue). I would be worried about some type of cancer or an infected cyst since it has been there so long with no resolution. If it was a simple abscess or insect bite it would have healed by now. A year is a long time.... I suggest you go ahead with the course of antibiotics your vet gave you and if no resolution I suggest you get a biopsy done of that tissue.
