My dog Charlie is medium. Why are harnesses too loose or tight?

would you say charlie’s a medium dog? i always have trouble finding a harness because a size small is way to small, but the medium somehow always ends up too loose. (some notes: i’ve measured him and he’s always in the medium size; on the last picture he’s obviously the smaller one ahaha). Pet's info: Dog | Mixed Breed | Male | unneutered
