Poor Mickey! It is difficult to say for sure with just a picture, but it looks like he has a little sore on the back side of his scrotum. I have seen these several times before and it is usually from playing rough outside, a little bug bite, or something similar that irritates the skin and the dog usually make it worse by licking or scooting on it. You could try some Benadryl by mouth twice a day (see link below) and some Benadryl spray and Neosporin twice a day. I would spray it first then apply the Neosporin once the spray drys. I would play a cone collar on him if he is licking it. If it isn't healing or getting worse, I would strongly recommend to have your vet take a look at it. Your vet will be able to determine the cause and get him some antibiotics and stronger allergy meds if needed. https://www.petcoach.co/canigive/dog/benadryl/#!
