My Chihuahua's vulva is swollen with a hole after nursing. Infected?

my Chihuahua gave birth to one puppy 5 weeks ago and is still nursing and she already has gone into heat. Today I noticed one side of her vulva was really swollen and she was licking it a lot. I left then came back home about three hours later and on the side that is swollen there is a pea-sized hole that is somewhat deep and it did look somewhat wet but now it looks like its kinda drying out a little but the swelling seems like its not getting any better. Pet's info: Dog | Chihuahua | Female | 6.3 lbs

I would be concerned she may have an infection causing her pain and discomfort hence the licking and self trauma. It is very early for her to be back in a true heat (although not impossible) and I recommend a full vet check as soon as possible to determine exactly what is going on
