My dog's scrotum is always red. How can I find a permanent cure?

Pet's info: Dog | German Shepherd | Male | unneutered | 3 years and 4 months old

he is always having this kind scrotal always red from last yr vet always prescribe medicine and antibiotics then it gets ok and few days later again same red as shown in attach photo and this time vet prescribed cephalexin along with itraconazole and trypsin bromelain RUTOSIDE so but I want permanent cure for this because giving such medicine everytime after few months will damage his kidney and liver which I dont want so what should I do and are there more dogs also whose testicle are red

The scrotum appears to be normal to me. Some dogs have a pink scrotum and it is not concerning. I would only be worried if scrotum was ulcerated, oozing, has scabs on it, etc. This does not look inflamed. If they have a specific time where they are red/inflamed you can use a hydrocortisone spray to help with the redness. The only permanent cure for scrotal dermatitis (which he doesn't seem to have, at least according to these photos) is castration with a scrotal ablation.
