Why is my cichlid fish lying on its side and struggling to swim?

my male cichlid is lying on his side on the bottom of the aquarium he has been sick for a year no matter what I do to treat him he isn't getting any better what should I do he has trouble swimming and staying upright I've been feeding him metronidazole haven't been feeding this for a year to him just for ten days /have a girl fish that seems to have something going on with her took her to vet said she had something hard inside her could it be a parasite

Thanks for reaching out about Tank. A radiograph or blood work could help provide answers. Radiographs could show a fluid/pus filled swim bladder. Injectable antibiotics could assist. There is little that can be done at home. Good luck.
