My dog has facial scabs and hair loss. Why isn't treatment helping?

Pet's info: Dog | Mixed Breed Small (up to 22lb) | Female | spayed | 1 year and 1 month old | 14 lbs

My adopted Pugalier is just over 1 yr, in the last month she has developed some skin issues on her face. It started as a sore and is now small circles of mini hair plugs falling out that feel like a raised scab in areas over her face, when you scratch it the hair falls out. Our vet didn't know what to do, but gave her 10 days of antibiotics, she did this in full and it hasn't helped, I then tried 10 days of anti fungal cream and wash that hasn't helped. Please see attached a pic of the hair plug

If antibiotics and antifungals have not helped, it is possible that Pickle has mange. The oral flea and tick preventatives simparica, Nexgard and Bravecto are often effective in treating demodectic mange and can be purchased from your veterinarian. If that doesn’t help, having a skin biopsy performed either at your veterinaian or with a veterinary dermatology specialist at a specialty hospital would likely be the next best course of action.
