I have take my maltese to vet three times and they said there doesn't seem to anything wrong. they gave me antibiotic ointment, helps a little. this all started after he got his yearly rabies shot. I feed him a raw- cooked diet and have for 3 years now. he will lick the area which of course makes it worse. I noticed today when I saw go the bathroom he was having a hard going and it was string not the normal size poop.

This area does look very red and inflamed - a common reason for such symptoms in an otherwise healthy dog is a food intolerance/adverse food reaction to something he is eating (possibly one of the meat sources). If he is in pain - a pelvic xray to rule out obstruction or growths may be considered also
