Why is my dog licking, losing hair, and has black skin spots?

My dog has been licking his tummy and started loosing his hair. I see 2 black holes and seems like something is in there stuck. I did the k9 advantix for 2 months but not helping. Would you please advise me what I can do for him? Any recommendations for the treatment or products? Pet's info: Dog | Shiba Inu | Female | spayed | 22 lbs

It looks like he has a severe dermatitis so he needs to be examined by a vet to determine the cause. He may need an antibiotic, anti-inflammatory, anti-histamine, and/or other medications and your vet can prescribe. At this point, I don't think over the counter medications are going to treat his problem.
