Why is my dog with CHF twitching after starting heart medications?

Updated On September 23rd, 2025

Pet's info: Dog | Mixed Breed Medium (23 - 60lb) | Female | spayed | 10 years old | 60 lbs

My dog was recently diagnosed with congestive heart failure. She was recently put on furosemide and benazapril. She seems to be doing much better, but she has recently started to make some jolting motions when she relaxes. I can also notice a slight twitch almost in sync with her heartbeat. Should I be concerned or does this seem like a normal side effect of medications? Thank you!

Hello. Furosemide is a diuretic & benazepril is a vasodilator, both of which are used to help deal with the effects of CHF. A twitch is generally not a side effect of either these two medications. This does sound a bit like a condition called synchronous diaphragmatic flutter or Thumps. Mostly this is a benign, idiopathic condition that often goes away on its own, but I would report it to your vet in case he may want to look at Lily's electrolytes like calcium, phosphorous & possibly even her parathyroid hormone levels. No emergency, just get with your vet at your earliest convenience. Thanks for using Petco Pet Education Center, formerly Petcoach.
