Why are my male puppy's testicles red and swollen, should I worry?

I would not worry about the swelling since they are the same size. The inflammation on the skin may be due to contact dermatitis and sitting or rubbing against something irritating to him. Clean the area with soap and water. You can then apply cortisone cream to help with redness and inflammation on the skin there. If it is no better by morning, then I would have your vet check it
