My senior cat returned home sick after missing. What should I do?

My older went missing 3 weeks ago but suddenly returned home tonight . Her eyes are dark and enlarged and she is all skin and bones , she also walking with a limp she is a 15+ year old rescue eating normal and using litter tray since returning but she was finding it hard to navigate i think she has gone blind . Pet's info: Cat | Mixed Breed | Female | spayed | 17 years and 11 months old | 10 lbs
