My dog won't take Cyclosporine for perianal fistula. Is it serious?

My German Shepherd has Perianal Fistula and he refuses to take his CycloSporine no matter how hard we try to disguise it. Will he die without it? Pet's info: Dog | German Shepherd | Male | neutered | 3 years and 5 months old | 88 lbs
