My cat reacts strangely when I touch his tail. Is he hurt or itchy?

Pet's info: Cat | Mixed Breed | Male | neutered | 12 years and 9 months old

So my cat has been weird when we touch near his tail. Sometime placing hand there will cause a reaction other times a hard scritch. Sometimes no reaction at all. He reacts like a female cat when aroused licking the air and somtimes his side. Never the same side. He made quite distinct howl when my sister tested him and it was the only time. I know his pain howls in the depth of my soul ( he nearly died twice when he was 4 and 5). Is he possiblity hurt or just my horny old man xP.

This behavior could mean Sake is itching back there (it's hard for them to reach that area) and you scratching it resolves the itch, or he could simply like being scratched there. It may feel good to him. Cats can be sensitive to being scratched in that area. I hope this helps!
