My old dog is suffering. Can I help him pass away humanely at home?

Pet's info: Dog | Shih Tzu | Male | neutered | 14 years and 9 months old | 18 lbs

I have my amazing dog Levie ( Shitz Tzu) 15 years. He has Cancer along with blindness, Arthritis, a heart disease, he also can't manoviure stairs, tumors in his mouth and 1 bad tooth. I know his time is close but we all want to say our goodbyes but it is soo hard . We don't want to see him suffer. We wanted to have his last day with all his family and be there until he passes. Do I have to bring him to the vet for this or is there something that I can give him example a pill so he can passaway

Thank you for submitting your question regarding Levie. I am sorry to hear that he has reached the end of his life. It is obvious how well loved he is. You will need to make an appointment with his veterinarian for euthanasia. There is not a humane method for doing this with any over-the-counter medications at home. He will need professional care.
