My old dog is suffering. Can I help him pass away humanely at home?

I have my amazing dog Levie ( Shitz Tzu) 15 years. He has Cancer along with blindness, Arthritis, a heart disease, he also can't manoviure stairs, tumors in his mouth and 1 bad tooth. I know his time is close but we all want to say our goodbyes but it is soo hard . We don't want to see him suffer. We wanted to have his last day with all his family and be there until he passes. Do I have to bring him to the vet for this or is there something that I can give him example a pill so he can passaway. Pet's info: Dog | Shih Tzu | Male | neutered | 14 years and 9 months old | 18 lbs
