Why is my dog's itchy ear crater not healing with medication?

Pet's info: Dog | American Staffordshire Terrier | Male | neutered | 6 years and 6 months old | 65 lbs

Dog has raised-edge circular area (like a crater) inside ear that is very itchy. Does not seem to be responding well to ointment and medication. Vet didn't know what it is. Live in mid-Hudson Valley, New York.

That is quite weird. I don't know what it is either, but would have a foreign body (something like a piece of a plant wedged in there causing irritation), resistant infection, or even a tumor, on my list of concerns. I would consider a culture and sensitivity test to make sure it doesn't need an unusual antibiotic, and then either biopsy the area or go to a dermatologist for a specialist opinion.
