Why is my dog lethargic, breathing heavy, and in pain every 2 weeks?

Pet's info: Dog | Keeshond | Female | spayed | 1 year and 2 months old | 135 lbs

Every two weeks or so, lasting for about 4 days, Eevee gets lethargic, heavy breathing, touching her sometimes makes her Yelp, lack of appetite, lots of heat, usually dry nose, lack of energy and sleeps most of the day, tail between her legs and head held low. I dont know what's going on and its scaring me. What could it be

I am sorry to hear about Eevee! This must be very worrisome to you! I would recommend having her examined by a veterinarian and having some comprehensive bloodwork done as a baseline. With waxing and waning symptoms such as this we do worry about things like Addison's disease https://veterinarypartner.vin.com/default.aspx?pid=19239&id=4951484 which is a problem where the body doesn't produce enough steroids. Most often we see it in dogs around 4 to 5 years old but it can happen at any age. We would also worry about some infections that can wax and wane, such as tick borne infections. Your vet can get a more thorough history and do a physical examination which may further help guide testing they may recommend or possible diagnosis.
