There are several things that could cause this like an urinary tract infection, prostate infection, bladder stones, an injury on the penis or inside the prepuce, a urethral prolapse and more. I think he needs to be looked at again by your vet and more diagnostics done. Your vet should be able to do a prostate exam, an urinalysis, and a thorough exam of the penis and prepuce.
