Why does my kitten poop everywhere, and her rear looks inflamed?

I rescued this poor little kitten when she was almost five weeks old . Since I've had her every where she lays she poops but in her sleep she uses the litter box she is very active and seems to be happy. But I don't think her back end looks right can some one please help

This area is very inflammed and looks sore. I recommend a veterinary exam to deworm her and check this area. She may need medication to soothe the skin and they can check to make sure her rectal opening is normal. It’s to hard to tell from just the photo. Cute kitten!
