My neutered dog's penis gets hard with a knot. Is it normal?

I have a German Shepard that was neutered 8 months ago. about 6 times that I know of his penis gets hard and a knot forms half way down it. If he is laying down he is unable to stand until it goes away. Did the vet miss something? Pet's info: Dog | German Shepherd | Male | neutered | 1 year and 3 months old | 85 lbs
