Dog fell off bed, limping, carpal pad hurts. Why isn't medicine working?

Pet's info: Dog | Shih Tzu | Female | spayed | 9 years and 8 months old | 16 lbs

Hi, can a dog injure their carpal pad if they fell off a bed? I took my dog to the vet since she's limping but they couldn't find anything wrong and gave her an anti inflammatory pain medicine but every time I touch the carpal pad she moves her paw away from me. She's been taking the medicine for a week now and there has been no change.

There is a possibility that she could have injured the bone below the carpal pad. However I am very concerned that her medicine is not relieving the pain at all. I think the next step now is to take her back to the vet for a recheck and x-rays. She may have serious arthritis, or even a partial or hairline fracture. Please take her back to the vet soon.
