Dog vulvar discharge: Pyometra? Do I need surgery or a second opinion?

My female dog was having discharge from her vulva then vet gave med for 10 days then told ultrasound then 2nd vet who did ultrasound said vaginitis only n gave vaginal pessary n did ultrasound after a week again n said infection there,surgery is required from ultrasound report.bt I feel Dr is very money minded(from his reviews,attitude).she initially was sometimes off food n sleepy bt now seems normal,active,eating okay.plz see ultrasound pics n tell uterus is visible? discharge is still there.

It's a little difficult to see for certain in the picture, but I do see a fluid filled structure in the photo near the urinary bladder that certainly could be the uterus. In your final picture the discharge on the ground does not look what I would typically expect from vaginitis. You may want to get a third opinion from a veterinarian if you're concerned about going forward with surgery. If it is a uterine infection it is unlikely to clear with antibiotics alone and even if it does the infection returns with the next heat cycle, so I typically recommend surgery whenever I am concerned about a pyometra.

    I would recommend to have surgery performed if it is the best recommended option. I would say that there is fluid in the uterus and increased shadowing, it is just a picture so difficult to fully comment. I know it is a surgery but it is very common for dogs to suffer with pyrometers as they get old, so it would prevent her anyway from getting it, hope this helps
