My dog won't stop scratching and chewing his tail raw. What can I do?

My dog has in the past month started to scratch and bite his backside, and now he is chewing his tail raw. He has had flea meds and given a bath and it got worse. I alao changed his food and used hot spot spray that seems to work for a few days but then he moves to a new spot to bite at. Please help. In photo he just did today while i was at work. Pet's info: Dog | German Shepherd | Male | neutered | 6 years and 2 months old | 90 lbs
