My dog is lethargic, disoriented, and acting aggressive. What's wrong?

My dog seems lethargic, maybe slightly disoriented and she flinches when hands come to her face. Headbutted daughter today and chewed a pen and got ink in her mouth. Also found someone tightened her collar to about 1 finger. Pet's info: Dog | Mixed Breed | Female | unspayed | 2 years and 9 months old
