What kind of turtle or tortoise did I rescue? How do I care for it?

I have no idea what kind of animal this is. Is it a tortoise or a turtle? My grandmother rescued it from a golf course and now she isn't able to look after him (or her?). I need to know what kind of habitat this little guy needs, and what diet he needs, please and thank-you! Pet's info: Amphibian | Unknown - Amphibian | Male
