12 week old puppy vomiting (who was just cleared at the vet as perfectly healthy) and refusing water (he did lick a little peanut butter which I did to test his willingness) nothing was in his vomit but it appears to smell very sour

It could be that Truffles ate something rotten that has upset his stomach leading to the vomiting. The vomiting can also be caused by a GI illness or a foreign body. It is concerning that he won't drink water, and if that behavior keeps up, he will need to see a vet right away to prevent dehydration. I can give you a home remedy to try on Truffles to see if that helps with the vomiting. Withhold food for 12-24 hours, then feed a bland diet of boiled white meat chicken (no bones) and white rice in small amounts several times a day for a couple of days. Then slowly add back in his normal diet. If the vomiting continues in the absence of food, he vomits the bland diet, develops diarrhea, refuses drinking water, starts vomiting water, or becomes very lethargic, he will need to see a vet right away.
