My Shih Tzu's tail hair is falling out. Is it an allergy or Cushing's?

I have a 3 years old shih tzu.A month ago I notice hair loss on his tail. Two weeks ago I took him to the vet and they said that ist probably a flea allergy. They medicated my dog with Teramil P, but I haven't notice a change. I wonder if it could be cushing. I read all symptoms. But he does not drink big amount of water and his pee is normal as always. He does not show a pot belly or anything, but the hair from his tail is falling. I read that cushing if common in older dogs and he is young.

It's very unlikely to be cushing's, he doesn't seem to have most of the characteristic signs. it's more likely to be an allergy, maybe not to fleas though. allergy meds such as apoquel should probably be attempted next.
