My old dog's severe muscle atrophy is worsening. When to say goodbye?

My dog suffers from muscular atrophy, she's been on steroids for a year and a half and we have up the dosage. She's an old dog but its gotten much worse recently, her head has no muscle on it and her eyes have sunken into the back of her head to the point where you can see the insides of her eyes.

I'm sorry that Cassie is suffering from muscular atrophy! Unfortunately, as time goes on, it does get worse despite treatment. Give the severity of her symptoms, in my opinion, I would evaluate her quality of life. This disease is painful to dogs, and it could be that her quality of life has reached a point with humane euthanasia should be considered. I know that's not something you would want to read, but when a dog is quite sick, it is a kindness to let them go without further pain and suffering. I work for a home euthanasia service, so I face this dilemma all the time with my clients. If you'd like to consult with me to discuss this further, I'd be happy to talk more in depth with you.
