My 11 month old female 6 lb dog's nipples are bleeding. Why? Should I be concerned?

Yes, I would be concerned that Bella's nipples are bleeding, as this is never normal. This could be an indication of an infection of her mammary glands, or this could be a sign of a false pregnancy. I would have her seen by a vet first thing in the morning.
