My senior Shih Tzu has Cushing's & is sick. When is it time to say goodbye?

Pet's info: Dog | Shih Tzu | Male | neutered | 12 years and 1 month old | 12 lbs

My Shih Tzu (12~15 yrs) was diagnosed with Cushing's Disease, urethra stones, and gastric wall hyperplasia. I took him to the vet because he began to puke, refuse food, and had minimal urination. The vet performed a retrograde catheterization, gave meds, and sent us home with an appointment in 5 days. I live in Korea but the vet spoke English fairly well and wrote me the attached document, I can see that Gizmo is not happy. I am wondering if this is it? Is it time to put him down?

Whatever decision that you make for Gizmo will always be the right decision. If Gizmo has not responded to the treatments and medications from his vet visit, I would recommend seeing the vet sooner than 5 days. He may need additional medications to control his symptoms. It sounds like his current issues are expected to be manageable with medications. However, there is a possibility that the stomach wall thickening could be cancerous, in which case Gizmo may not feel better, even with medications. You may discuss pain medications or treatments such as acupuncture with the vet to help with Gizmo’s comfort.
