My dog has a smelly neck wound with worms. How to calm him for care?

my dog has a good size hole at the back of his neck that has worms in it and smells for 5 days now, i've tried spraying him with screwworm spray but not much goes in the hole because he's running away. What can i give him to let him sleep or calm him down. Going to the vet is a no no. Pet's info: Dog | German Shepherd | Male | unneutered | 7 years and 9 months old | 86 lbs
