Unspayed dog has pain. Is it heat or infection? Should I see a vet?

Pet's info: Dog | Maltese | Female | unspayed | 3 years and 7 months old | 4 lbs

Doctor David. Please pray that is not a uterine infection. I'm really praying and hoping that it's not something serious. Could it be that it hurts her because she is about to be in heat? Could I take her for an exam even when she's in heat or should I wait?

You should definitely take her for an exam ASAP, as she isn't spayed and is suffering from pain in the abdominal area, a uterine infection would be the first suspicion, but it could of course be something else such as a gastrointestinal problem. Heat cycle by itself is not likely to be the cause of such a pain.
