My dog is losing weight, has lumps, and dark pee. Should I worry?

Pet's info: Dog | West Highland White Terrier | Female | 13 years and 4 months old | 6 lbs

she drinks water a lot. She gets excited to eat or when people have food even now. She has lost weight recently,started to have darker pee, barely will drink water unless it is in my hand.We took her to the vet in 2016 & they said, she had a yeast infection, she had a small lump of fat on her right side back hip. She now has a small lump on her right front leg, she is skinny. You can see her bone structure and feel some of her spine.This has happened before but she always gained the weight back

You need to take her back to the vet ASAP in order to get this checked, first the vet will need to do a blood test to try and find out what is causing those symptoms, the lump will need to be checked and analyzed as well.
