How to heal dog hot spots so I can finally remove the e-collar?

Pet's info: Dog | Shih Tzu | Female | spayed | 12 lbs

My pup developed hotspots under her ear after ear inflammation. The vet sent her home with 4 days of oral steroids, steroid creme and ear drops. I kept an e-collar on for six days as per the vet. But within 5 minutes of taking it off, my pup scratched and reopened the hotspots. So I put on more creme and did another 5 days of the collar. They looked healed when I took the collar off but again within minutes she opened them. What more can I do to get the collar off and the hotspots healed?

Hi. Poor Lilly. Sounds like you have done the best you can. Lilly may need oral antibiotics at this point to get it to heal. Until m9nday, you could try putting a triple antibiotic ointment on the area until you get it examined.
