It is also possible that a bladder stone or something else causing a urethral obstruction was causing him to strain and expose his penis. Regardless of the cause, inability to urinate is a medical emergency and you should see an emergency veterinarian right away.

There appears to be some major inflammation at the tip of the penis, most likely from an injury or excessive licking. My best recommendation to you would be to have Taco examined by your veterinarian. I would bring a urine sample to he appointment if you are able to catch one. It depends on the amount of blood and in how much pain he is in. Consider he is small so even a patch the size of an adult person hand is quite a lot for him. If the blood is little drops and he seems bright in himself, no other concerns, you could wait but in all other cases I would call the vet now. I hope he gets better soon !

Bite wounds are serious because these can easily result in more trauma than what seems apparent and also in an infection. If the os penis is fractured this can be very serious as it may have lacerated the urethra. It's possible that he may have a congenital issue, such as a narrow urethra or abnormally positioned/functioning ureters. It would be unusual for a dog Romeo's age (as well as when he was younger) to have prostate issues, but again, congenital issues are most likely. Without knowing why this is occurring, it's impossible to say what may help to improve this problem. At this point, your best bet is to take him to the vet for an exam and a urinalysis (just to rule out signs of infection - just to cover bases), some imaging. Your vet may recommend x-rays of his abdomen to check his bladder position and for the presence of stones, as well as an abdominal ultrasound to visualize his entire urinary system. The membrane on the penis is not designed to be exposed to the air for an extended period of time so it can become dried out and damaged when exteriorized for an extended. Of time and become susceptible to infection or even swelling that is severe enough to prevent urination. The emergency room will evaluate him and try to get the penis back in its sheath.
