My dog yelps and has a swollen bottom when pooping. What's wrong?

Pet's info: Dog | Lhasa Apso | Male | unneutered | 3 years and 5 months old | 15 lbs

My Lhasa Apso male called Chewie always yelps when going for a poo, his bottom ring piece is a bit swollen, I always keep his bottom clean and sometime I put hemorrhoid cream on it, I have taken his to our vets pets at home, but they so he seemed to be OK, Chewie is now 3 and I 1st took his to the vet at 6 mths old

I would recommend to take Chewie back to the vet for a prostate exam and an x ray. You want to make sure that the colon looks normal and there's no obvious reason why it or his prostate would be causing any pain while having a BM. The doctor will check his anal glands as well to make sure they are ok. Take in a fresh stool sample to make sure it is to too dry/firm and causing a problem. You may get some answers.
