Why is my 5-month puppy's head back, not eating, but drinking water?

My dog is 5 months. Her head is extending backward like she's falling asleep or falling over but she's awake. She won't eat. Wants water tho. What is this? Pet's info: Dog | Mixed Breed Small (up to 22lb) | Female | unspayed | 4 months and 28 days old | 7 lbs
