This sounds like a urinary infection or a prostate problem. If he’s having trouble urinating this is an emergency as he could die if there is a urinary blockage. Regardless with the pain and crying I recommend a veterinary exam soon. They will run a urinalysis and check his prostate for enlargement. They may run blood work and do an abdominal X-ray to check for bladder stones and the size of his prostate. If it’s an infection they will prescribe antibiotics. His penis looks inflamed, possibly infected, you will need to take him to the vet to get it checked, he will need to wear an e-collar in order to prevent from him to continue licking his penis and getting it worse, he will also need an antibiotic course.

Buddy may have injured himself. I recommend getting him seen by a vet as soon as possible for an exam. Don't let him lick it excessively and do more damage to himself, if necessary, get an inflatable e-collar from PetsMart to keep him from being able to reach to lick himself.

Some dogs do have a protrusion above the urethral opening on their penis and it's often lighter colored than the rest of the tissue. This is normal. However, if you are noticing him licking more than normal, this often indicates discomfort. Young male dogs, especially if they are not neutered, often get infections in the prepuce and this can cause excessive licking and often an increase in discharge. Male dogs can also get bladder infections which can also cause increased licking. Since Jonesy cannot talk and tell you why this is happening, I recommend to have him examined this week. Paraphimosis is the inability to retract the penis back into the sheath, so if he is able to retract it then he doesn't have that. If it ever gets to the point where he is unable to retract it then it is a medical emergency and he needs to be seen by a vet as soon as possible.
