My dachshund's bad breath & dirty teeth won't improve. What now?

I have a miniature dachshund and her breath smells bad and her teeth are dirty. I brush them, add water fresheners, and even a gel freshener. It doesn’t seem to be working. What can I do? Pet's info: Dog | Dachshund miniature | Female | spayed | 9 lbs
